  • Medication list if you are taking any medication (including dosage and times)

  • List of existing medical conditions

  • Previous medical reports, if available (e.g., lab results, X-rays, medical letters, hospital discharge summaries, etc.)

  • Photo ID and e-card, if available

  • Referral letter from your GP or specialist, if available

  • Registration form (please print, complete, and bring it with you if this is your first visit to our practice)
